Before Your First Appointment
Paperwork and Set-up
You will receive an email inviting you to create your client portal in Sessions Health, Foundry’s electronic medical record (EMR) system. You will be prompted to sign a consent form and add a credit card on file. This is the system where you will manage appointments, reminders, assessments, etc.
*PLEASE CHECK YOUR SPAM IF YOU HAVE NOT RECEIVED THE EMAIL
For clients billing through insurance, you will be prompted via email and text to create a client profile in Headway Co., fill out their consent forms, and set up billing. You will only use Headway for BILLING purposes only.
THOSE BILLING THROUGH INSURANCE WILL HAVE TWO PORTALS - The one described above, with sessions health, and one with Headway for billing.

The address for the office is 400 Mailtand Ave, Altamonte Springs, FL 32701
The building where the office is located is called The Lighthouse.
This building houses and provides space for multiple non-profit organizations. Supporting the community is part of Foundry’s values. If there are voices, music, laughter, or even if you hear folks doing martial arts in the large room next to the lobby - you are still in the right place!
When you arrive you can choose to wait in the lobby, or in your car.
The main door faces the street and will lead to the lobby.
To “check-in,” text (407-205-8270) that you have arrived and where you are waiting - or if you are running late. The therapist will meet you in the lobby or the side door nearest their office based on your text.

If you prefer to wait in the lobby:
There is water and coffee in the lobby, and creamer in the small refrigerator - feel free to help yourself.
The bathroom is on the right at the end of the hallway.
If you prefer to wait in your car:
You can park in the spaces around the south side of the building (including the grass) and the therapist will come from the side door to let you in.

Telehealth
If your appointment is online, you will receive a link in your email reminders. If you do not, you can text (407-205-8270) directly and receive the link before the session.
